How do you approach building and maintaining a quality management system?

I start by mapping the processes that have the most direct impact on product quality and customer satisfaction, then design controls at the points in those processes where failure is most likely or most costly. I build the QMS around the ISO 9001 framework because it provides a structure that works across industries and is auditable, but I adapt the documentation depth to the complexity of the operation rather than generating paperwork for its own sake. Once the system is in place, maintenance is about two things: keeping it current when processes change and making sure it is actively used rather than sitting in a folder. I run internal audits twice a year and I treat them as a learning exercise, not a compliance check: the goal is to find gaps before an external auditor or a customer does. I also track the corrective action backlog: a quality system where non-conformances are raised but never closed is worse than no system at all.

How do you investigate a quality failure or customer complaint?

I use an 8D or equivalent structured investigation process for any significant quality failure. The first step is containment: before I investigate why it happened, I make sure we have stopped the problem from continuing. That means reviewing any suspect product in the pipeline and holding it pending the investigation result. Then I move to root cause analysis using a 5-Why or Fishbone approach, depending on the complexity. I never stop at the first plausible cause: in my experience, the first answer is almost always a symptom rather than the root cause. Once I have the root cause confirmed, I design a corrective action that addresses it structurally, not just procedurally: changing a process step or adding an automated check is more durable than reminding people to be more careful. I verify the effectiveness of the corrective action at 30 and 90 days before closing the 8D. I also look for similar processes where the same failure mode could occur and apply preventive action proactively.

How do you build a culture of quality in an organisation that has historically treated quality as a policing function?

Changing a quality culture starts with changing what quality is seen as doing. If the quality team is only visible when something goes wrong, the association is negative: quality means problems and blame. I break that by getting quality people onto the production floor during normal operations, helping teams solve day-to-day problems rather than only auditing them. I also shift the metric emphasis away from defect counts towards process capability: a team that understands how to keep their process within control limits is doing quality work, not a team that is simply being monitored for mistakes. I involve production teams in designing their own control plans: when operators own the quality check on their own process, they understand why it matters. I also make the business impact of quality failures visible: the cost of a single customer return, a line shutdown, or a regulatory hold is usually far larger than most frontline teams realise, and making it concrete changes the conversation.

How do you manage supplier quality?

I treat supplier quality as part of my quality system, not a separate function. My approach has three layers. The first is qualification: before a new supplier goes live, they complete a quality questionnaire, I review their certifications, and for critical materials I do an on-site audit. I set minimum requirements based on the risk level of what they supply. The second layer is ongoing monitoring: I track incoming inspection failure rates by supplier and by material, and I have a defined threshold that triggers a supplier quality review meeting. The third layer is improvement: when a supplier has a recurring quality issue, I work with them directly on root cause and corrective action rather than just switching suppliers, because switching is expensive and a well-managed improvement often delivers a better outcome than a new supplier who has not yet been tested at volume. I also share my quality requirements and control plan expectations clearly at the contract stage, because most supplier quality failures I have seen stem from unclear specifications rather than supplier incompetence.

Tell me about a time you successfully reduced the defect rate or quality failure rate in an operation.

At a food manufacturing site I joined, the end-of-line reject rate was running at 4.3%, which was double the industry benchmark for our product category. I spent the first two weeks mapping where in the process each type of defect was occurring rather than looking at the total number. This analysis revealed that 67% of rejects were coming from a single process step: the sealing station, which was generating seal failures due to temperature variation. The root cause investigation found that the temperature control thermostat had a response lag that was creating out-of-spec conditions for roughly 8 minutes in every 90-minute production cycle. We replaced the thermostat with a faster-response model and added a real-time temperature alarm. Within six weeks the reject rate at that station had dropped from 3.8% to 0.6%, pulling the overall end-of-line rate to 1.1%. The improvement was worth approximately £180,000 per year in reduced waste and rework.

Describe a time you managed a major customer complaint or recall situation.

A key retail customer raised a complaint about foreign body contamination in a batch of our product. It was a metal fragment, which meant we had to treat it as a potential safety issue immediately. Within two hours of receiving the complaint, I had initiated containment: all product from the same production run was placed on hold at the customer's distribution centres and at our own finished goods store. I notified our senior leadership and the relevant food safety authority as required. The source investigation found that a wearing component in one of the filling machines had shed a fragment during the run. I worked with the engineering team to inspect all machines of the same type, replace the worn component across the fleet, and modify the maintenance schedule to catch this failure mode earlier. I communicated proactively with the customer throughout: daily updates during the investigation, a formal close-out report within ten days. The customer stayed with us as a supplier, partly because of how we handled the communication.

Give an example of a time you had to push back on production or commercial pressure to maintain quality standards.

A production manager wanted to release a batch of product that had failed a visual inspection check against our specification, arguing that the defect was cosmetic and would not affect function. The commercial team backed him: the order was urgent and the customer was waiting. I held the hold and presented three pieces of evidence to the leadership team. First, the specification was set at customer request, not internally, so releasing out-of-spec product without customer approval was a contractual risk. Second, we had a history with this customer where a previous cosmetic concession had led to a returns dispute. Third, I had contacted the customer directly to explain the situation and offer a 48-hour delay: they accepted the delay. The product was regraded, the conforming portion was released the same day, and the non-conforming portion was held for reprocessing. The production manager understood the rationale once the customer had confirmed they valued the standard.

How do you use statistical process control in your quality work?

I use SPC as a process management tool, not just a monitoring tool. For any process parameter where variation directly affects product quality, I establish control limits based on the natural variation of the process (using at least 25 data points before setting the initial limits) and plot the parameter on a control chart updated at defined intervals during production. I train operators to respond to out-of-control signals, the Western Electric rules, without waiting for a quality engineer to interpret the chart: the goal is that the person running the process detects drift before it produces out-of-spec product. When a process consistently runs near its control limits, that is a signal to investigate the process capability, not just watch the chart. I calculate Cpk for all critical parameters and target a minimum of 1.33 for established processes and 1.67 for processes supplying safety-critical applications. The most common mistake I see with SPC implementation is setting limits too wide, which means the chart never signals even when the process is drifting.

How do you approach risk assessment and FMEA in your quality work?

I use FMEA (Failure Mode and Effects Analysis) at two stages: during new product or process design and when investigating significant quality failures. In design FMEA, I involve a cross-functional team including engineering, production, and quality to systematically work through every process step and identify potential failure modes, their effects, their severity, and the current controls in place. The output is a risk priority number that tells me where to focus preventive action. I update the FMEA whenever a process change is made: an out-of-date FMEA is misleading because it implies controls exist that may have been removed or changed. For process FMEA I pay particular attention to the detection rating: a low-severity failure with a low detection probability can still cause a significant customer problem if it gets through at volume. I also connect the FMEA to the control plan, so every identified risk has a corresponding check in the production process.

How do you prepare for and manage an external quality audit or regulatory inspection?

My approach to external audits is to run the business as if the auditor is already there, so there is no difference in what I show them and what happens on a normal day. That means the QMS documentation is always current, non-conformances are always being closed within the agreed timescale, and internal audit findings are actioned before they accumulate. In the two weeks before a scheduled external audit, I run a mock audit using the same checklist the external body uses, and I address any gaps I find. I prepare the team by briefing them on the format of the audit and reminding them to answer questions directly and honestly: experienced auditors spot evasion immediately, and it creates more scrutiny, not less. On the day, I accompany the auditor during the floor walk and act as a facilitator rather than a defender: if they find something, I thank them for it and document the action on the spot.
